Patient Forms
If you are a new patient, please visit our Patient Forms page to download the new-patient bundle. Printing the forms, filling them out, and bringing them with you lets us attend to your medical needs more quickly than completing them on arrival. Please contact us if you have any questions.
Payment Options
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. If your treatment plan requires more comprehensive work and you would like to discuss financial arrangements, feel free to contact our office.
